If you use Simply Accounting to send your Relevé 1 slips to Revenu Québec on magnetic media (diskette, CD-ROM, or DVD), you must complete and submit form ED-400-V, Description of the RL-Slip Data File, with your magnetic medium. A copy of the form can be downloaded from Revenu Québec's Web site at www.revenu.gouv.qc.ca/eng/publications/ed/ed-400-v.asp. Enter the authorization number below on the "Revenu Québec authorization number" line in the Software section of the form.
Enclose the form with the magnetic medium and send the package to:

If you are transmitting your Relevé 1 slips in a data file over the Internet, do not submit the paper version of the ED-400-V form to Revenu Québec. You can provide the required information when you access Revenu Québec's RL-slip electronic transmission service.
